Dermal Fillers
Dermal fillers are injectable treatments designed to restore lost volume, smooth fine lines, and enhance natural contours of the face. As skin ages, a decrease in collagen, elastin, and fat leads to volume loss, sagging, and fine lines. Dermal fillers address these specific changes by replenishing volume, smoothing wrinkles, and restoring facial contours for a more youthful appearance. At our Park Avenue office in New York City, we use only FDA-approved fillers including hyaluronic acid fillers (Juvéderm®, Restylane®, RHA® Collection), as well as calcium hydroxyapatite (Radiesse®) and poly-L-lactic acid (Sculptra®).
- Restore youthful facial volume in the cheeks and midface
- Smooth wrinkles and folds (nasolabial folds, marionette lines)
- Augment lip shape and volume
- Soften under-eye hollows
- Enhance chin and jawline
- Fill temple hollowing
- Facial balancing and improving facial symmetry
With strategic placement of conservative volumes in the appropriate anatomic plane, the result is natural, elegant, and tailored to your unique features. The goal is to look refreshed, without looking overdone.
Two major classes of dermal fillers include hyaluronic acid fillers and biostimulatory fillers.
- Hyaluronic acid (HA): Attracts and binds water, instantly restoring volume and hydration. The result is immediate augmentation and lifting of the tissue. These fillers typically last 12 months, with some showing clinical benefit up to 2 years. Over time, your body naturally degrades these fillers and the tissue returns to its baseline. Biostimulatory fillers promote your own collagen production using different substances. Unlike HA fillers, these fillers can take up to 3 months for your body to produce its own collagen, so full results are typically seen later.
- Calcium hydroxyapatite (CaHA): Provides immediate lift while stimulating collagen production.
- Poly-L-lactic acid (PLLA): Works gradually to stimulate your body’s collagen, improving skin structure over time.
Ideal for men and women noticing early volume loss, etched lines, or who want subtle contour enhancement without surgery.
- Cheeks – Restore lost volume and lift the midface. As we age, the fat pads in the cheeks may shrink and shift downward, resulting in noticeable volume loss and hollowing. Treatment of the cheeks can restore youthful contour and fullness while also improve the undereye area and the nasolabial folds.
- Undereye / Tear troughs – Soften dark circles and shadowing. Loss of volume under the eyes can lead to a hollowed, tired appearance. Undereye fillers can reduce shadows in this area, leading to a brighter, more rested look.
- Nasolabial Folds / Smile Lines – Reduce the appearance of deep lines around the mouth and lower face. As volume is lost in the midface, these lines can deepen and become more noticeable. Restoring volume in the cheeks along with subtle filler placement in the nasolabial folds can lead to a balanced, natural result.
- Marionette lines / Oral Commissure – Lifts the corners of the mouth to prevent a downward pull and sad appearance. Filler is strategically placed to create a lifting vector, providing structure and support to the corners of the mouth. Restoring volume to this area can also camouflage the appearance of early jowls.
- Lip Borders and Volume – Fillers are commonly used to restore shape and volume to the lips, and can also be used to correct minor asymmetries or imbalances between the upper and lower lips. As we age, the lips become thinner and definition is lost. Different types of fillers can be employed whether the goal is to redefine the borders or plump and hydrate the lips.
- Chin and Jawline – Enhancing the chin and jawline can provide a sharper delineation between the lower face and neck, imparting a healthier, stronger appearance. This also reduces the appearance of jowls and better frames the overall symmetry of the face. Adding projection to the chin can create a more balanced proportion to the face.
- Temples – Restoring volume to fill temporal hollowing balances the upper face and softens the transition between the upper third and midface. This can also help support the lateral brow to keep it lifted.
During the consultation, Dr. Hibler will evaluate your individual facial anatomy, review your aesthetic goals, and assess whether filler is the right solution for your needs. Often times, dermal filler is combined with other strategies (neuromodulators, tissue tightening devices, and lasers for a more holistic approach to facial rejuvenation. Careful selection of the type of filler, volume to be injected, and advanced knowledge of facial anatomy results in the most natural look without appearing overdone.
Injections are performed using a combination of fine needles and cannulas (long, thin, blunt-tipped flexible tubes). By using a combination of needles and cannula, patient safety and strategic placement of filler is achieved. Many dermal fillers contain lidocaine to minimize discomfort during the procedure.
Mild swelling or bruising may occur and typically resolves in days. Rare risks will be reviewed during consultation.
- How long do fillers last? HA fillers last 6–18 months; CaHA 12–18 months; Sculptra up to 2+ years.
- Can fillers look natural? Yes — in expert hands, results are subtle and elegant.
- Are fillers reversible? Hyaluronic acid fillers can be dissolved with hyaluronidase if needed.
